Rules of Clinic
|
- You must have the appropriate permit to be seen in the Clinic - not the Hall Pass but "Teacher's Referral to Principal of Nurse" Form. Obtain from teacher.
- No student will be seen without a permit except in extreme emergencies, such as asthma attack, insulin reaction, fainting, bleeding, seizure or serious injury.
- Students are not to come to the Clinic at the end of the period (last 10 minutes) nor between classes. Report to your next period teacher for a permit. (The teacher whose class you are missing must give permission.)
- The Clinic is also open during entire lunch period. Students may visit without a note.
- At all times, the student must sign in and out on the Nurse's Daily Registry - showing time he/she arrived and left the Clinic. IF YOU DON'T SIGN IN, THEN YOU CANNOT PROVE THAT YOU HAVE BEEN TO THE CLINIC.
- Students who feel ill and go home without going through the Clinic and Attendance Office are considered truant and are subject to disciplinary action.
- You must be evaluated by the Nurse to be excused from school via the Clinic. THE NURSE WILL CALL THE PARENT.
- Should a student call their parent to pick them up before coming to the Clinic, they will be sent to their Asst. Principal for approval to go home.
- You will not be seen in the Clinic if you are not in dress code, including shoes.
- Report injuries to the Nurse. Sports injuries should also be reported to your Coach.
- If you are feeling faint or light-headed, do not leave the classroom alone to come to the Clinic.
- If you have a Chronic Health problem, see the Nurse if you have not done so already.

Students are not allowed to remain in school with the following conditions:
- Fever of 100.4 or more
- without medication
- Suspected contagious condition
- Vomiting
- Diarrhea
- Cannot remain comfortably in class
- Undetermined rash
- Personal products are available in the Clinic for $.25.
- If you have any problems or concerns, come and talk with the Nurse. Referrals will be made as indicated.
